Athletic Clearance Process

All student-athletes must complete the athletic clearance process before participating in practices or competitions at Pueblo Academy of Arts.

Option 1: Visit Pueblo Academy of Arts

Athletic clearance packets may be picked up and returned to PAA. Most required forms can be downloaded and printed using the links below. 
Please note: School offices may have limited hours or be closed during portions of June and July.

Option 2: Visit the D60 Athletics Office

If the PAA office is closed, families may contact the District 60 Athletics Office for assistance.

Option 3: Download Forms Online

Families may click the required forms, download them, and print them using the links below. Completed forms should be returned to PAA.

• PREPARTICIPATION PHYSICAL EVALUATION (This medical history form should be retained by the healthcare provider and/or parent. This form is valid for 365 calendar days from the date signed. Only turn in page 4 to the PAA office. It must be signed by the parent/guardian and a healthcare provider.)
• Parent Guidelines Agreement
• Student Activities Agreement
• Athletic Fee Waiver Form (Must also complete the survey at My School Apps to see if your family qualifies to have the fee waived.)
• Emergency Contact Card (Only available in the PAA Office or the D60 Athletic Office)
• D60 Application to Participate in Athletic Programs (Only for athletes who go to a D60 Middle School that does not have athletics, Charter Schools, Private Schools, Online Schools, or Contiguous Boundary Students. Only available in the PAA Office or the D60 Athletic Office)

Athletic Participation Fee

The athletic participation fee is $50 per sport, per athlete, and is due by the first day of practice. Fill out the Athletic Fee Waiver Form to see if your family qualifies to have the athletic fee waived. (Must also complete the survey at My School Apps)
